PHYSICAL THERAPIST ASSISTANT
Description:
Physical Therapist Assistants are professionals that provide care as needed for our patients in a home setting.
You will support the Physical Therapist in meeting the needs of patients and work with other professionals to manage patients suffering from CHF/COPD or in need of anticoagulation therapy due to total hip/knee replacement.
Average visits require the PTA to assess patient, provide teaching, manage risks, and work with other disciplines to improve patient outcomes.
Requirements:
- Licensed in good standing by the State of Texas
- Experience with geriatric patients a plus, and
- Orthopedic / ICU / CCU / Telemetry / ER or related experience preferred
- Able to substantiate improvement in patient outcomes
RECERTIFICATION SPECIALIST
Description:
Recertification Specialist reviews and verifies the documentation needed for completion and signing of patient orders by referring physicians to support admission, 60-day recertification or discharge. This position is responsible for coordinating communication of orders between providers and the company's staff regarding assigned patients to ensure patient status is accurately maintained in accordance with company procedures and high-quality patient care.
Requirements:
- Licensed to practice professional nursing in Texas and may be either an RN or an LVN
- Minimum of three years' experience as LVN/RN; one year experience in a similar position preferred
- Demonstrates understanding of the cardiovascular and pulmonary disease processes, as well as diabetes and other major illnesses which are monitored by the home health agency field Case Manager.
- Ability to communicate effectively in English, verbally and in writing, with people of varying backgrounds inside and outside of the agency.
- Has knowledge of insurance company rules, regulations and benefits.
- Must be able to work independently.
- Maintains a professional appearance and decorum.
- Maintains Continuing Education Units (CEUs) pertaining to disease process, case management or home health.
- Knowledge of MS Office Suite, MiSys Software, or demonstrates ability to learn these systems.
CASE MANAGER COORDINATOR
Description:
Case Management Coordinator acts as the primary liaison between the agency and insurers on claims for non-Medicare patients to ensure the agency is reimbursed for services rendered.
Requirements:
- Licensed to practice professional nursing in Texas and may be either an RN or an LVN
- Experience as case manager in hospital discharge planning, home health, insurance case management or equivalent positions
- Orthopedic / ICU / CCU / Telemetry / ER or related experience preferred

RN CASE MANAGER
Description:
RN Case Manager provides cardiovascular home care. Performs home visits for an average of 6-8 patients a day for approximately 25-45 minutes each. Visits require RN to run assessments, provide teaching, manage anticoagulation, and treat HF / COPD as indicated.

MEDICAL SOCIAL WORKER
Description:
The basic function of the medical social worker is to provide assessment, treatment, case management and referral to community resources for physically ill patients of the agency. Services will be provided to the patients in their own homes. The medical social worker will document all services provided to the patient and family. The goal of services is treat all psychosocial issues that may impede patient recovery.
Requirements:
- Must hold a Bachelors Degree in Social Work, Master Preferred, from an accredited University
- Must be licensed by the Texas Department of Health as a Licensed Social Worker
- Must have 1+ year of experience in assessing and providing social services in a medical healthcare environment
- Must hold a valid driver's license, liability insurance and be physically able to provide services to patients in their home environment

Home Health Aide plays a vital role as part of the care team, monitoring the patients between nursing visits, reporting changes in the patient’s status, and assisting patients with personal care, including light housekeeping, in accordance with care plans provided by supervising nurse.
