Description:

Safe Choices RN provides specialty nursing services to help establish a successful transition from hospital to home.

Visits patients in their homes for approximately 25-45 minutes each. Visits require nurse to perform assessments, provide teaching, and manage transition processes, especially medication management.

Interacts with various disciplines to ensure patient outcomes are met. Team members include Geriatric Care Manager (MSW or LCSW), Certified Nurses Aides, Physical Therapists, and Occupational Therapist.

FIELD CASE MANAGER RN

Description:

RN Case Manager provides cardiovascular home care. Performs home visits for an average of 6-8 patients a day for approximately 25-45 minutes each. Visits require RN to run assessments, provide teaching, manage anticoagulation, and treat HF / COPD as indicated.

OFFICE NURSE

Description:

This full-time RN clinical coordinator tracks, processes and manages anti-coagulation readings from Field RNs for orthopedic and cardiovascular patients within company guidelines to ensure necessary follow-up treatment is communicated for high-quality patient care. The Anti-Coagulation/Intake Coordinator reviews new patient contacts to confirm appropriate service and accurately develops new patient information sheet coverage to ensure all assigned patient classes have no break in care. In addition, this RN enters Medicare billing codes/diagnosis for referrals. Also makes assignments and coordinates optimal treatment for Medicare patients with Case Managers and Admissions staff.

PHYSICAL THERAPIST ASSISTANT

Description:

Physical Therapist Assistants are professionals that provide care as needed for our patients apist in meeting the needs of patients and work with other professionals to manage patients suffering from CHF/COPDin a home setting.
You will support the Physical Ther or in need of anticoagulation therapy due to total hip/knee replacement.
Average visits require the PTA to assess patient, provide teaching, manage risks, and work with other disciplines to improve patient outcomes.
Full-time, part-time and PRN positions available.

RECERTIFICATION SPECIALIST

Description:

Recertification Specialist reviews and verifies the documentation needed for completion and signing of patient orders by referring physicians to support admission, 60-day recertification or discharge. This position is responsible for coordinating communication of orders between providers and the company's staff regarding assigned patients to ensure patient status is accurately maintained in accordance with company procedures and high-quality patient care.

Description:

Case Management Coordinator acts as the primary liaison between the agency and insurers on claims for non-Medicare patients to ensure the agency is reimbursed for services rendered.

MEDICAL SOCIAL WORKER

Description:

The basic function of the medical social worker is to provide assessment, treatment, case management and referral to community resources for physically ill patients of the agency. Services will be provided to the patients in their own homes. The medical social worker will document all services provided to the patient and family. The goal of services is treat all psychosocial issues that may impede patient recovery.

Description:

Occupational Therapist works with other field staff to ensure cardiovascular and orthopedic patients' needs are met within the occupational therapy discipline scope. Part-time and PRN positions available.
