At the family's or physician's request, we provide a comprehensive evaluation with action plan to seniors and their families that best promotes the senior's health and ensures a safe living environment. Click for more info
Safe Choices services include:
- Evaluation of the senior's functional abilities and limitations by an MSW/Geriatric Care Manager and an RN in the current living environment.
- Recommendations for the level of care and services necessary to keep the senior safe and medically compliant.
- Family meeting to discuss evaluation findings and develop a plan to implement recommendations.
- Counseling for seniors', family members', emotional and psychological issues related to the aging process.
- Physician coordination to assist the senior to maintain medical compliance.
- Interventions can be initiated to ensure placement or services necessary to keep the senior safe.
